Personal Section:

Not as bad as yesterday, but still painful. Was able to stand for short periods of time for cooking. Pain has shifted from my feet to my ankles. Also still isolated pain in parts of my fingers. I’ll be glad when I can get through this current arthritic wave. Its starting to ebb as I believe the anti-inflammatory meals are having an effect.

Eating Section:
Had my anti-inflammatory breakfast. Was able to cook my standard brunch and then an afternoon snack of walnuts. So a good day.

Exercise Section:

Still hobbled by arthritis. Hope to start walking again tomorrow.

Day 8/10 & 9/10 of ReFeed

Weight gain: Unknown for last 2 days due to arthritic pain and then dead batteries in the weight scale. Total weight gain: 22lbs

BGL: Not taken due to arthritic pain in hands for both days
Fasting: No Fasting for both days
Exercise: Rest
Energy: :sob: (:sob: In Pain, Tired, :ok_hand: Ok, :muscle: Strong)

Personal Section:

Friday night was bad as I had arthritic swelling in my left achilles and left middle finger. Only got 2 hours of sleep due to the pain. As a result all I could do was nap and rest all day Saturday. Luckily its the weekend. Last night, the swellings gone down enough I was able to catch up on my rest. Still the left hand hurts and while the swelling and pain has gone down in the left foot, I had to have assistance in cooking this morning for my Beef & Vegetable stew that should last me for several days.

I’m thinking I may have to delay the 30-day workout phase for 1 to 2 days due to this arthritic attack. It shouldn’t impact my deadline as I had a buffer of 2 days before my June 12th deadline.

Eating Section:
Had my anti-inflammatory breakfast both days. However, I confess that since I couldn’t cook yesterday, I bought McDonalds for the family. I haven’t had MickeyDs since last summer. As a result I had gas and diarrhea. That’ll teach me! LOL

Exercise Section:

Still hobbled by arthritis.

Personal Section:

In spite of hot ā€œwetā€ and topical treatments, the joint inflammation was getting worse and spreading. I had to re-assess my perception of reality to create a long-term solution to this re-occurring problem which posed a serious threat to my goal of ā€œ6-pack at 60ā€. Here are the steps I undertook:

FIRST: I had to accept the fact that in my zeal to maintain my ā€œmedication freeā€ status had led me to a mis-diagnosis of the type of arthritis I was suffering at end of both of my 21-day fasts and refeeds.
It’s NOT osteoarthritis but a re-occurrence of my gout. The redness and swelling around the joints in my hands and feet was visual evidence I could no longer refute. The specific reason for the mis-diagnosis was because I didn’t want to get back on my anti-gout medication.

SECOND: Accepting it was a re-occurrence of gout I could start my investigation. It would begin with why did it re-occur ONLY at the end of my 21-day fasts and NOT at the end of my 40-day? Here is where doing my daily journal entries and stats keeping on my spreadsheets helped. The data led me to do more detailed scientific digging and this is the conclusion I come up with.

INVESTIGATION CONCLUSION:

  1. By the end of my 40-day I had dropped from a starting weight of 278lbs to 197lbs. A loss of 81lbs. That seems like a lot. However 20lbs had only been gained during April 2018 to August 2019 due to the daily insulin injections. As I remember weighing in at 258lbs the day I was first proscribed the daily insulin injections,

  2. When I got hit by my unexpected health crisis in September of 2010 that led me to being bedridden for 5 years and the other subsequent health issues I believe I was around 203 to 211lbs range at the time.

  3. Therefore, I had not started digging into my deep visceral belly fat until I began the 21-day fasts. Studies done by NIH (National Institutes of Health) have shown that visceral fat have stored preinflammatory cytokines IL-6 & IL-18 that can raise the uric acid level in the body. For myself, even though I was still in a fasted state this caused the arthritic symptoms of stiffness BUT not the gout symptoms, It was ONLY during refeeds after the 21-day fasts that it became full blown gout attacks when I ate foods rich in purines that when broken down in the stomach raise the uric acid level in my body. Its like lighting a match in a gas station. It also explains why I did not suffer from gout after the 40-day and its subsequent refeed since my uric acid level was showing normal on the blood panel taken shortly at end of that long fast.

SOLUTION:
My treatment protocol is based on my investigation and this excellent YouTube series from John Hopkins Medicine Rheumatology Division. Here are 3 of the 6:

STEP 1: Immediate Pain Relief
Ibuprofen 600 MG tablet ever 4 hours. Took my 1st around 9pm. Immediate relief. Also stopped feeling so cold as body relieved of fighting inflammatory pain released energy for other bodily functions. Also stopped urinating every hour to hour and a half as body felt less compelled to excrete excess uric acid causing the pain. Took the 2nd at 1am. Slept 8 hours with only having to get up 3 times to pee. Also ordered refill of ant-gout medication through Walgreens auto system.

This morning was able to stand unassisted, and make my usual 2 meals for the day. By this early afternoon able to walk unassisted.

Took remaining 2 ibuprofen tablets at 7am & 11am. Followed up with doctor’s office they authorized refill prescription because its a 90-day supply. Got authorization. Confirmed with Walgreens who will have medication available for pickup after 2:15pm PST.

STEP 2: Short-term solution
A. Avoid foods with purines until ALL gout symptoms gone.
B. Take daily medication whose purpose is to suppress uric acid production from foods processed in the stomach (Allopurinol).
C. Extend current ReFeed phase due to end with 30-day workout phase scheduled to start tomorrow by 2 more days. Workout schedule now slated to start Thu 4/23 and final 21-day fast delayed to May 23rd with it ending on same day as primary doctor visit on 6/12 to review blood panels taken 6/5.

STEP 3: Long-term Solution.
The good news: Once I get rid of the remaining visceral fat and curing my CKD I will finally rid myself of gout long-term. I’ll take allopurinol on days I eat purine heavy foods as a precautionary measure.

The bad news: I’ll have to keep ibuprofen around for ā€œbreak glassā€ short-term relief emergencies. I’ll continue to take allopurinol during the remainder of the refeed and during the entire 30-day workout phase.

I plan to run this by the endocrinologist at USC in my weekly status report, due today, to see what he thinks.

OTHER NEWS:

  1. Well I dropped 3lbs since I last weighed myself. Probably all pee weight! LOL
  2. Nice to see being able to maintain my BGL at normal through all this craziness.

Eating Section:
Had my anti-inflammatory breakfast. 2nd meal had no food high in purines.

Exercise Section:

Feeling better but not confident enough to do my usual 1 mile circle walk.

Fasting Section:

Now that I have correctly diagnosed my ā€œarthritisā€ issue and implemented a solution that appears to be working, I’m shooting to re-start fasting with an 18:6 beginning after my last meal finished around 12:30pm earlier today.

Writing Section:

Going to write an article on the little journey with my ā€œarthritisā€ problem with more scientific references for review by the endocrinologist at USC.

Public Speaking Section:

On hold until I schedule my next speech.

Day 11/12 of Refeed
Weight gain/loss: 0 Total weight gain: 19lbs
BGL: 105 - 5pts above normal
Fasting: 15:9
Exercise: Rest

Personal Section:

Well a nice compliment of validation from the endocrinologist at USC:

ā€œAs for you deductive investigations and treatment protocol, it all makes sense! Very impressive! Definitely stay well hydrated and keep the ibuprofen on hand. Keep it up!ā€

Last night some final (I hope) tiny gout flare ups in the left ankle and right elbow. The right elbow is good now. The left ankle still creaky but feels more stable, The left wrist still a bit stiff while the inflammations in the left are shrinking but still red and tender.

Weight plateaued at 198lbs. That’s just 1 pound more than the low I reached at the end of last year’s 40-day. It would be great if that’s my body’s new set point.

Not freaked about today’s BGL being 5 points higher than normal. As long its just a daily blip and NOT a trend.

Eating Section:
Had my standard anti-inflammatory breakfast. 2nd meal was my homemade Veg & Beef stew. Since I’m taking Allopurinol again I’m not worried about the bits of beef causing another gout attack.

Exercise Section:

Feeling better but the creaminess of my left ankle cautioned me to take another day of rest.

Fasting Section:

Happy to restart fasting again. Missed how it makes the 1st meal of the day so satisfying and the daily feeling of accomplishment.

Writing Section:

The extensive writing I did for yesterday’s daily journal and my weekly status report to the USC endocrinologist took a lot out of me. I also posted about my experience in the FB group as a case study in the importance of daily journaling and stats recording to allow health professionals you’re working with to truly serve you reach your goals.

Public Speaking Section:

I think I have the topic of my next speech! LOL

Day 12/12 of Refeed
Weight gain/loss: 0 Total weight gain: 19lbs
BGL: Normal
Fasting: 20:4

Personal Section:

Mis-timed my medication. It says to take ā€œTake 1 tablet by mouth at bedtime.ā€ Problem is there is an assumption my last meal was 3 to 4 hours ago. NOT 11 hours ago! As a result the medication wasn’t there when my digested food raised my uric acid level and I suffered a brief and painful gout attack. Luckily it lasted only a hal hour as the medication kicked in. Unfortunately, it halted the healing progress I was making. Specifically, I had the outside part of left ankle swell up. I could hobble around though with some pain. I did hot ā€œwetā€ treatments and that drove the swelling down. I’ll do it again after journaling and one more time this evening as Workout phase is scheduled to start tomorrow.

3rd day in a row I’ve plateaued in weight even after completing an IF 20:4. Confirms this IS my new weight set point.

BGL dropped 20 points back into normal. Yesterday just a daily blip like I suspected.

Eating Section:

Had my standard anti-inflammatory breakfast. 2nd meal scrambled eggs w/sweet onion, fried salt/vinegar potato slices and walnuts.

Exercise Section:

Hopefully final day of rest

ReFeed De-brief:

Goals:

  1. Feed stem cells & minimize weight gain. Target weight: 195lbs
  2. 2 meals per day
  3. Play by ear but try for minimum 1 mile walk
  4. Time restricted eating.

Results:

  1. Goal: 195lbs Actual: 198lbs. 3lbs over goal
Happy with result due to complications from re-occurrence of gout.
  2. Stuck to plan during non-gout days. 6 of 12 days. Satisfactory due to gout complication.
  3. Gout and underestimating energy drain during early part of refeed due to stem cell conversion demands. 3 of 12 days: 1 mile walk.
  4. Time restricted eating schedule: 6 of 12 days. Satisfactory due to gout complication.

The re-occurrence of gout was a downer as it also pointed out that my clinging to my status of being ā€œmedication freeā€ led to my misinterpretation of the data to make an initial misdiagnosis. It turned into a positive experience as it forced me to marshal all of my recently acquired scientific knowledge to conduct a thorough investigation and create a treatment protocol. Both investigation and protocol reviewed and approved by the Clinical Assistant Professor at USC’s Medical School.

Underestimated how much food my body needed during the 1st 5 days of the refeed for stem cell conversion. Result was I ended eating some ā€œbadā€ foods because of they’re being the most calorie dense. Will add more potatoes due to they’re being the #1 food on the satiety index.
